Patient will need to provide a current insurance card & a photo ID at every visit. *Please complete and bring to your first appointment; Registration Form General Patient History Form Please print these forms from our web site complete and bring to your 1st appointment. Forms are also available at the office if you do not have printer availability. Insurance Information Medical card(s) along with co-payments (via cash, check, visa, mastercard) Workman's comp information (if work related) Auto insurance (if auto related) Date of injury (This may be a specific date or an on going problem) Any previous testing or records if available: X-rays, CT, MRI, etc… Copayments Please know we are required by insurance companies to collect any co-payments at the time of your office visits (via: Cash, Check, Visa and MasterCard *please note we do not except discover*) Prescriptions If medication is needed you must first be a patient of Lakes Orthopaedics Specialists. Refill requests need a 48 hour notice and will not be refilled after 3:00 p.m on Thursdays, Holidays or weekends. No Insurance If you are currently without medical insurance you will be expected to pay for your service at the time of your visit. We offer a 30% discount for payments made in full. Please call the clinic manager (651-389-0181) about any payment and insurance concerns you may have. Please note a payment of $300.00 down for your visit at time of service. Treatmemt If any medical imaging is ordered please schedule a follow up appointment 3 to 4 days following your scheduled test to go over the results and to determine the next plan of action. Hospitalization If your treatment requires hospitalization our providers have privileges at Fairview Lakes Regional medical center.
